well some states dont allow body piercers to use anethstetics if they are not registerd to use them.. ie certified nurses.. but i really wouldnt think that recieving a transdermal would be too terible painful.. what gauge, width are you starting.. plus its a rite of passage you should just take it the way it is.. kinda makes it worth it..

he's talking about a transscrotal and not a transdermal.
Usually there are no anesthetics involved, if you want it, go through with it. That's my opinion.
As far as I know it is done by cutting from the front to the back of your scrotum with a scalpel and stitch it up. So yeah it is a very painful procedure as I would imagine.

Does it have to be cut with a scalpel?
Why can't a needle be used?

A transscrotal is not! a piercing.
It has to be done with a scalpel and sutured together, for it to heal.
If you just pierced it with a needle, and threw jewellery in, it would not heal due to the type of tissue it is.

sorry didnt mean to say transdermal but the answer still depends on the state regulations on the use of anethstetics.. most of the time anyway..

Just a minor clarification. I have seem transscrotal piercings can heal. But, when you start stretching you are much more likely to "bust." Basically, you're forming a fistula where there isn't really tissue.

Suturing just ensure there is a tight bond between the two sides and a smaller seam that is less likely to "bust."

Though, I've seen a few transscrotals scalpelled in with no sutures merely a teflon piece and some super tight O-rings and they turned out just fine.

Scalpel and suture is definitely the safest bet though.

Get it done properly.
You could use a topical anesthetic like EMLA or something.. its over the counter in Ontario, not sure about where you are.
