i never had mine pierced before it was done.  scalpelled and stitched. i ahve a tiny bit of regrowth that im going to get cut again. but i would speak to the body modificator rather then have us guess on here

It probably doesn't matter if you've got your tongue pierced at a regular size. This is why it's good to stretch to at least 6mm, preferably more along 8-10mm or so. That way there is an anchor at the bottom of the split and regrowth is less likely to appear.
My piercer cuts with a scalpel then brands it with electro-cautery pen, no sutures.
